Clinical Observership Insights with Italian Plastic Surgery Authority Dr. Giulia Lo Russo
Executive Summary: What defines true natural breast augmentation? This column presents Dr. Wen-Chi Huang’s in-depth reflections on her visit to Italy to observe Dr. Giulia Lo Russo, one of the early pioneers in the use of Luna XT™ anatomical implants. Through this high-level international exchange, the essence of surgery is reconsidered: how a mature surgeon uses precise restraint and systematic management to reduce clinical variability, allowing natural-looking breast outcomes with long-term stability to become more predictable.
To continuously advance my clinical thinking, I traveled to Florence for a specialized observership at the clinic of Dr. Giulia Lo Russo, a highly respected plastic surgeon. The purpose of this visit was to engage in an in-depth exchange regarding her long-term clinical experience with GC Aesthetics Luna XT™ anatomical implants.
Dr. Giulia is a highly experienced specialist in Italian plastic surgery. Since obtaining her board certification in 2002, she has served for many years as an attending surgeon at the University Hospital of Florence, accumulating extensive experience in both breast reconstruction and aesthetic breast surgery. Within the international academic community, she is particularly recognized for complex gender-affirming chest surgery, including Top Surgery-female to male chest contouring as well as male to female breast reconstruction and augmentation.
When facing highly complex baseline anatomy, she is able to create proportionally balanced and fluid contours with a natural appearance. She is also among the earliest surgeons worldwide to adopt Luna XT™ anatomical implants. After our in-depth exchange, I realized that the value of this visit extended beyond technical maneuvers and reached a deeper level of clinical decision-making.
During my conversation with Dr. Giulia, she shared an insight that resonated deeply with my own practice: “A good outcome is already determined before entering the operating room.”
Although the statement may seem straightforward, it captures a core truth of breast augmentation. Every patient who enters the consultation room has unique variables, including soft-tissue quality, anatomical structure, skin elasticity, lifestyle, and expectations after surgery.
A surgeon’s value lies in the ability to identify these variables pre-operatively and integrate them into a comprehensive system of planning. The physical act of surgery is the final step in executing those decisions; what truly influences success is pre-operative clinical judgment.
In today’s medical aesthetics environment, natural breast augmentation is the goal many patients pursue, yet the concept is often oversimplified.
In Dr. Giulia’s aesthetic vocabulary, naturalness does not simply mean doing less. It is an overall presentation built upon proportion, structure, and dynamic harmony. This requires a profound respect for human tissue characteristics, as well as deep understanding and meticulous observation.
The highest level of aesthetic judgment lies in precise calibration, where each clinical decision becomes a measured form of control. Naturalness means achieving the right degree of intervention. This pursuit of balance helps create a breast profile that remains harmonious both at rest and during movement.
Another important observation from this exchange was how a surgeon’s clinical decision-making evolves with accumulated experience.
In the early stages of training, surgeons may tend to add more maneuvers to secure an outcome. With experience, however, one gradually learns to avoid unnecessary tissue manipulation. Reducing surgical trauma is an important factor in post-operative recovery and natural-looking outcomes.
Therefore, mature clinical judgment is reflected in the ability to identify the precise surgical pathway, avoid areas of risk, and achieve more reliable outcomes with less tissue disruption.
When discussing post-operative safety and complications, which are central concerns for surgeons, Dr. Giulia offered a broader perspective: complications are often not the result of a single isolated error, but the cumulative outcome of the entire medical workflow, including patient selection, pre-operative planning, surgical execution, and post-operative care.
This serves as an important reminder: the long-term results and safety of breast augmentation must be built upon a stable, consistent, and standardized system.
Although this observership did not introduce an entirely new surgical technique, it helped me reorganize several important clinical perspectives:
The essence of surgery lies in the close integration of clinical decision-making and technical execution.
A natural outcome comes from continuous refinement of experience rather than simple conservatism.
A mature clinical strategy involves optimized and more precise surgical steps.
Long-term stable outcomes do not rely solely on an individual surgeon’s technique, but on the establishment of a comprehensive medical system.
Dr. Giulia’s long-term clinical experience demonstrates the temporal value of the Luna XT™ implant. Years of clinical use across Europe have shown favorable structural stability and aesthetic performance over time. The properties of the implant provide an important foundation, while individualized surgical design gives that foundation clinical direction. By incorporating mature international perspectives into the details of patient care, we aim to support natural, stable, and reassuring outcomes over the long term.
This international exchange in Italy further reinforced my conviction: across different physical conditions, our role is to identify the most appropriate and reassuring solution for each woman who enters the clinic with expectations for change.
