從條件到結果:Luna XT™ 水滴型隆乳的結構美學思維

近年來,國際間的醫療技術交流日益緊密。此次我非常榮幸受邀參與GC AestheticsGCATrain the trainers 國際講師培訓課程,有機會近距離觀摩 Dr. Antonio Di Vincenzo 的臨床教學與手術操作。

作為 GCA 的官方核心講師,Dr. Antonio 長期專注於乳房美容手術,特別是在解剖型植入物(Anatomical implants)的臨床應用、乳房結構調整以及胸型比例設計上,具備相當成熟且系統化的經驗。他同時也是目前全歐洲 Luna XT™(璐娜水滴)植入物手術量最多的權威醫師,參訪當天更密集示範了 4 台高難度手術。

在這次緊湊的手術觀摩中,我最深刻的感受是:

在過往的臨床經驗中,許多人認為理想的胸型往往需要受術者具備一定的先天優勢,例如良好的皮膚彈性、足夠的乳房基底條件或均勻的組織分布。

但在這次觀摩中,我看到了另一種強大的可能性:

透過乳暈切口進入,靈活搭配乳腺結構調整與解剖型植入物的應用,即使原本條件並不理想、甚至面臨嚴重產後萎縮或不對稱的個案,仍然可以被成功引導至更接近客觀美學比例的結果,呈現出自然的動態與觸感。

手術的價值,可以只是簡單地放大患者既有的優勢;但更高階的價值,是藉由經驗與核心技術,重新建立和諧的比例與結構。

技術的本質:降低對條件的依賴

這次交流帶給我一個很明確的體悟:

在臨床初期,我們往往認為好的結果來自良好的條件;

但當工具升級之後,真正的目標,是讓結果不再過度依賴條件。

換句話說,手術建立在對解剖結構的理解與調整之上,使乳房結構朝向更理想的比例發展,並選擇符合需求的假體,這也是影響隆乳效果自然度的重要關鍵。

在整個觀摩過程中,可以強烈感受到 Dr. Antonio 的手術建立在一套可複製(Reproducible)的邏輯系統之上。

Executive Summary: Is an ideal breast augmentation outcome only achievable in patients with favorable baseline anatomy? In this column, Dr. Wen-Chi Huang shares her personal insights from the international GC Aesthetics (GCA) Train the Trainers masterclass. Through close observation of one of Europe’s most experienced surgeons in the use of Luna XT™ anatomical implants, a refined surgical perspective emerged: how precise technique and implant pocket design can reduce overreliance on a patient’s baseline anatomy and support natural-looking breast outcomes aligned with objective aesthetic proportions.


In recent years, international medical and technical exchanges have become increasingly interconnected. I was deeply honored to be invited to the GC Aesthetics (GCA) Train the Trainers international faculty program, which offered an invaluable opportunity to closely observe the clinical teaching and live surgical demonstrations of Dr. Antonio Di Vincenzo.

 

As an official core faculty member of GCA, Dr. Antonio has long dedicated his practice to aesthetic breast surgery. He has developed mature and systematic expertise in the clinical application of anatomical implants, breast tissue adjustment, and breast proportion design. He is currently regarded as one of the highest-volume surgeons for Luna XT™ anatomical implants in Europe, and on the day of my visit, he demonstrated four consecutive complex live surgeries.

Throughout this intensive surgical observation, my most important takeaway was this: through sustained refinement of experience and technique, surgeons can help patients with different baseline anatomies move toward a more ideal and natural-looking breast profile with greater consistency.

 

In his surgical practice, Dr. Antonio has long utilized the periareolar approach and integrates it as a key element of his overall surgical strategy.

Through this incision, he gains direct and precise access for breast tissue adjustment while creating an appropriate implant pocket, allowing the breast structure to be redistributed and optimized. Within this operative logic, breast augmentation is not limited to implant placement; it also involves structural adjustment and proportional design of the entire breast profile.

This approach demonstrates a highly active role in breast shape design. The final outcome is shaped through surgical planning and execution rather than being determined passively by the implant shape alone.


Anatomical Implants: A Tool for Problem-Solving, Not the Sole Answer

Regarding implant selection, Dr. Antonio extensively uses the Luna XT™ anatomical implant. Based on long-term clinical follow-up, he recognizes Luna XT™ long-term stability and clinically reliable performance.

Nevertheless, even a well-designed implant must be supported by sound surgical planning and precise technique. With extensive experience, Dr. Antonio uses meticulous tissue adjustment and pocket design to help the implant function appropriately in the intended anatomical position. This prompted an important reflection: the same implant can lead to very different outcomes under different surgical strategies. Although this differs from the transaxillary endoscopic breast augmentation approach that I routinely specialize in, it offers valuable clinical reference when managing highly challenging and complex breast cases.

In previous clinical experience, many have believed that an ideal breast profile often requires favorable baseline characteristics, such as good skin elasticity, an adequate breast base, or even soft-tissue distribution.

However, this observership demonstrated another important possibility: through the periareolar approach, combined with flexible breast tissue adjustment and the use of anatomical implants, even patients with less favorable baseline conditions, including severe post-partum involution or asymmetry, may be guided toward an outcome closer to objective aesthetic proportions, with natural movement and palpability.

The clinical value of surgery may begin with enhancing a patient’s existing advantages; at a more advanced level, it lies in using experience and core technique to rebuild harmonious proportions and structure.


The Essence of Technique: Reducing Dependency on Baseline Anatomy

This exchange gave me a clear realization: early in clinical practice, we often attribute good outcomes to favorable baseline anatomy. As tools and techniques evolve, however, a surgeon’s true capability is reflected in reducing excessive dependence on those baseline conditions.

In other words, surgery is built upon a deep understanding and adjustment of anatomical structures. By guiding the breast structure toward more ideal proportions and selecting an implant that matches the patient’s needs, surgeons can better support a natural-looking breast augmentation outcome.


Consistent Outcomes Stem from Reproducible Technical Logic

Throughout the observation, it was evident that Dr. Antonio’s surgical execution is built upon a highly reproducible logical system.

Regardless of variations in patient anatomy, his operative steps and clinical decision-making principles remain highly consistent. This consistency reflects the pursuit of medical quality: surgical success should not be an isolated occurrence, but a standard that can be reproduced with stability over time
